New Patients and Patient Portal

Thank you for choosing Community Medical Center (CMC) for your healthcare needs.  Here you will find patient info/ FAQs that will help you better understand, and how to get the most out of your office visit. 

  

All forms are available and filled electronically on iPads in the office. It is a must for new patients to come in 30 minutes early for the first visit to update their records in our system electronically and effortlessly. 


You must sign up and utilize the "Patient Portal" to access your direct link to CMC. You can message directly to the concerned staff/ provider and get quicker response rather than trying to reach by phone. You can ask questions, request refills, request to schedule an appointment, review your records and results, see your bills and carry out certain other useful activity from the portal. 


Paying bills online has been deactivated for obvious security reasons. However, You can pay your bills over the phone.

Make the Most of your Visit

The key to a great doctor visit is to be prepared and to ask questions that you need answered. 


Bring: 

  • Please bring your valid photo ID. 
  • Do remember to bring Primary and Scondary insurance cards.
  • A detailed list of any symptoms or problems that are prompting the visit.
  • A list of all medications, vitamins, and supplements that you are taking.
  • A list of questions you want to ask about your health, so you don’t forget.
  • Inform your doctor of any changes to your health history, such as weight loss or gain, modifications to diet, new exercise program, altered sleep patterns, etc.

Ask:

  • Your doctor to explain things in a way you understand.
  • When you will get the results of any tests that are performed.
  • If you need help managing your care between different providers or specialists.

What should I expect at my first appointment?

You will meet with your Doctor who will review your information and perform physical exam. We encourage your feedback to ensure you receive the best possible treatment.  

What ages do you treat?

We have the staff and expertise to treat every member of your family. 

What Insurance do you accept?

We accept most insurance programs including but not limited to;

If your insurance is not listed here, give us a call and we’ll go over your policy with you.

What type payment do you accept?

In addition to most medical insurance payments, we accept VISA, Mastercard, American Express, Discover and cash. 

What if I don't have insurance?

No Problem — We have special discounted rates for patients without insurance. Our rates are almost always less than what you would have paid in monthly insurance premiums. 

When should I arrive for my appointment?

New Patients, Please arrive 30 minutes before your appointment time. This will allow you plenty of time to fill out any necessary paperwork (all done on iPads electronically) and record your primary and secondary insurance details to avoid any billing hassles. Also get your pharmacy detials as most prescriptions are sent electronically.


Follow up Patients to arrive 15-20 minutes before appointment time, depending upon the amount of changes required and co pays etc.

What happens if I need to cancel my appointment?

Because of the individualized nature of the services that we offer, our Medical Center providers prepare treatment rooms and services in advance for every client. However, we know that things sometimes happen.

So, if you are unable to keep your appointment, please provide us at least 24 hours’ notice (48 hours’ notice for specific treatment services).

CMC reserve the right to charge for appointments not cancelled within this time frame.
